I agree to make payments on the specified dates and the agreed amounts stated on the preferred way of payment. For any cancellations, there is a fee of 35% of the remaining of the class (conditions may apply) I understand the consequences that will be brought against me if the contract is violated.

I grant Story Institute my permission to use the photographs taken for any legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content. Furthermore, I understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
